Chronic Pain Management
A referral is required from your family physician or referring specialist. You will be contacted as soon as we receive the referral to fill out our mandatory intake form. The intake form provides the doctor with important information that helps direct your care. During your initial visit, the doctor will discuss your treatment plan with you. After each visit a report will be sent to your referring physician to provide some guidance in your care. We offer a shared care model where the primary care physician remains central in your care.
EMG Testing
We offer on-site EMG testing. A referral for this test from your family doctor or specialist is required.
